Critical hit  

Dating back to Dungeons & Dragons, most RPGs have this mechanic. Simply put, every action, whether a melee attack or a spell, has a chance at exceptional far beyond the normal range for its damage or healing. This chance usually increases via some game mechanic as the character grows.
