Certain items are tagged for extra potion belt slot flag. If you look at the raw data on Lucy Girth of Torment, you'll see a potionbeltslots entry, but Lucy doesn't correctly parse this data. However, 13th floor's raw data does correctly parse this information: Girth of Torment.
There is no information on the item display window to give an indication of this feature, it's only known either equipping the item, and noticing the change, or via the raw item data.

We are trying to get some bp ess for guild members, and (i have stopped counting) apx. killed 10-15 of these and twice has it dropped. Every single time the stupid Girdle of Pain has dropped. VERY Common drop if you are looking for it. I would bet money that you get it 1st try!
With so many ways to die, why choose just one?

I've found him to spawn at the same spot on server resets and after killing center stage mobs. Have never had him spawn anywhere else.
This guy spawns alot. not sure the loc but its on my map. It either came from Mapfiend or raspers loc. Very common.

Has about 415k HP's, average hit is about 2600, around 4k max. Our tank took about 125k dmg in just under 2 min

He is not a static spawn. As in he can spawn eslewhere in zone.
Location one. Go to the stage area at the very bottom of the library as fast easy as youc an get. Now the 2 PHs are at the top of the ramp. Theres 1 mob on one side and 3 on other his PH is one of the 3. Then down on the stage in the middle of it an imperial scourge is his PH in the very middle of the stage area. Ive gotten him to spawn multiple times in both spots.

Go east and drop into the pit to the very bottom, then take the north hallway and stay right. You should run by his spawn point near the southeast room.

Also drops Phased Seminal Incandessence.
This mob is yellow to 83. So at least on the spawns I've seen, he's not 89. That level doesn't even make sense given the tier of Kaesora Library.
All the named I've seen in both Kaesora zones are level 86.
