Apparently she takes off soon after you enter the room; I looked away and she was gone, off to get her dumb a$$ killed.
Also, the grate unlocked for me as well. Must be from the Hole key as a previous poster mentioned.

Just for the record, when i accidentally killed her, she took 1.5 hrs to respawn, or 90 minutes. If she paths strange, and goes to zone line and dissappears, hail her before she scoots, then retrun to her spawn point, hail her again and she gives u item. At least thats what happend to me.

from the drop down enter the castle got through first door and then forward to next door, continue down the hall until you see broken doors enter there should be some water there and one mob. Enter the water open the grate and continue down to the npc. When hailed she does one of two paths, one is to the spot you need for epic, and the other is into the water and seems to be a bugged pathing.

grey con at level 43, red con "a spirit of the old city" in the room with her, I got past the gate fine, possibly because I have Hole key on my /keyring?

I spoke to her as a berzerker of the 43'rd lvl bumped into her on accident, but she wouldn't speak with me, not enough experiance /sigh
She is on the other side of the locked underwater grate. I will post the exact /loc as soon as I get back to my notes.
